Medicare and Special Needs Plans – Tailoring Coverage To Your Needs.

Our Medicare expertise is aimed at helping seniors and eligible individuals navigate the complexities of Medicare and find the most suitable coverage options for your healthcare needs. Medicare is a federal health insurance program for people aged 65 and older, as well as certain younger individuals with disabilities.

In addition, Medicare Special Needs Plans focus on assisting individuals with specific healthcare needs, including those with low income or Medicaid eligibility. We navigate the complexities of these tailored coverage options. These plans are designed for those with chronic conditions or unique health requirements, offering specialized benefits and support.

Types Of Coverage

  • Medicare Part A: Provides hospital insurance coverage, including inpatient hospital care, skilled nursing facility care, hospice care, and some home health care services.
  • Medicare Part B: Offers medical insurance coverage for outpatient services, including doctor visits, preventive care, medical supplies, and some diagnostic tests.
  • Medicare Part C (Medicare Advantage): Medicare Advantage plans are offered by private insurance companies approved by Medicare. These plans combine Part A and Part B benefits and often include additional coverage, such as prescription drugs, dental, and vision services
  • Medicare Part D: Provides prescription drug coverage to help reduce the cost of medications not covered under Part A or Part B.

Chronic Special Needs Plans (C-SNPs)

C-SNPs are designed for individuals who have one or more chronic or disabling conditions. These plans provide specialized healthcare coverage and services tailored to manage and treat specific chronic illnesses, such as diabetes, heart disease, chronic obstructive pulmonary disease (COPD), or HIV/AIDS.

Dual Eligible Special Needs Plans (D-SNPs)

D-SNPs are designed for individuals who are eligible for both Medicare and Medicaid (known as “dual eligibles”). These plans provide coordinated care that combines Medicare and Medicaid benefits to help manage the comprehensive healthcare needs of low-income seniors and people with disabilities.

Institutional Special Needs Plans (I-SNPs)

I-SNPs are designed for individuals who live in a nursing home, long-term care facility, or require nursing care at home. These plans provide specialized care to meet the unique needs of residents in institutional settings

Institutional Equivalent Special Needs Plans (IE-SNPs)

IE-SNPs are a type of Medicare Advantage plan designed for individuals who require a nursing home level of care but are still living in the community. These plans provide healthcare services tailored to meet the complex needs of these individuals while they remain in their own homes

Programs For All-Inclusive Care For the Elderly (PACE)

PACE provides a comprehensive service package for older adults who need a high level of care but prefer to remain in their homes rather than move to a nursing home. PACE programs coordinate and provide all necessary medical and supportive services, including primary care, hospital care, prescription drugs, home care, adult day care, and more.
